This is the Template of Evaluation Questions, you can use or modify to follow up with your training participants to see what impact did the training have on their performance in their role.
This Evaluation should be sent out to the participants at least 6 months after the completion of their training.
It is important to follow up a couple of months after the training to really measure its success. Read more about why here.
